Horrible decision. Really let me down and created more unnecessary drama at the most crucial time for my father’s life, as well as his caregivers. Will never refer to anyone going through this transition period. Hospice is about easing pain and burdens, not blaming and finger pointing.

I love Brighton Hospice! They have been the best thing for mom and the family. The care she has received is beyond the best we could have ever expected. Our CNA Lela and our RN Julie are the best to say the least. The open communication is beyond anything we have experienced on the current journey with mom. They are so kind caring and compassionate with mom. Highly recommend Brighton to anyone looking for this service!! They are truly the best!!! Thank you Lela, Julie and Jennifer!!!

I just want to say that Brighton Hospice, Indiana was amazing with my Mom and her last months. Above all they were amazing for me. The support was outstanding, even hard to express how much that means to me. There were several people involved, one of which named Jennifer Price. She was the first that we met, and of course, immediately felt so comfortable and at peace. She was so loving, caring, compassionate, comforting, during an awful time of hard decisions. She brought ice cream to my Mom several times something that she loved very much and enjoyed! They say they’re angels among us and she’s one of them. I have no doubt in my mind she loved my Mom! The extra things she went out of her way to do for my Mom to make her happy and smile! Doing this brought such a comforting feeling when I couldn’t be around Jennifer usually came by or checked on Mom for me. Also want to mention Michael he had several spiritual moments with my Mom. He came and studied the Bible and prayed with her.
He also shared with me every time he was with my mom he knew for a fact that God was in the room with them. For him to share that with me is priceless, and I will never forget that kindness. There are so many more Angels from Brighton Hospice Indiana that I would like to say “Thank You!” From the bottom of my heart, it will never be forgotten and forever more appreciated! My mom was a patient of Brighton Hospice. I had a lot of issues with Brighton Hospice. My first issue was, instead of calling and coming over to do an assessment on my mom, the nurses would call and say, do you want a visit today. It made me feel as if they did not want to come out for a visit nor did they want to do an assessment on my mom. I spoke with the manager Demetria who stated, “we know families need their privacy and we try to give it to them”. Families do not sign up for hospice to get their privacy. We put our loved ones on hospice to get extra helping hands. Each family is different. If hospice do their jobs correctly, call and set up an appointment with family, and then give the family the right to say we do not need a visit today? Hospice should not be calling and asking the family, do you want a visit today? It made me feel uneasy about the whole process. The cna’s were horrible. Some of the cna’s did not know how to use a hoyer lift. I had to train them. One cna name Keisha was good at her job, but her attitude was horrible. She would come into my home and ask personal question about myself. She would ask me if I knew her husband. She never gave me enough supplies for my mom. When I ask for more gloves, she would tell me management monitors the gloves and did not give them enough to hand out to the families. Then she told me, why do you need gloves our family members do not use gloves”. I had to tell her I use gloves because my mom is incontinent and I want my gloves. I would run out of gloves and adult diapers and had to order them from Amazon. Once they switched our cna, I was able to get the proper amount of supplies without running out. One night my mom blood sugar was over 500. She was unresponsive. I called the after hour Brighton Hospice answering service and Michaela answered the phone. I explained to her that my mom’s blood sugar was over 500 and she was unresponsive. Instead of calling the doctor on call, she said to me, there is nothing we can do about it. I had to tell her to call the doctor. As soon as I hung up, my phone rang and it was Michaela again. She said the doctor said it was noting she could do. There was no way she could have called the doctor that fast. Brighton Hospice had previously discontinued my moms insulin. Thank God I still had insulin at home that I was able to give to my mom. Michaela called back again and then said the doctor wanted me to give her a blood sugar pill. I ask her, how am I suppose to give my mom a blood sugar pill and she’s unresponsive? After I told Michaela I gave my mom insulin, she had another on call nurse to call me. That on call nurse said to me I’m all the way on the other side of town, do you still need a visit as if their protocol was to avoid home visits at all cost. If I didn’t take matters into my own hands, my mom probably would not have survived that night. The next day someone in management called about the situation and all she did was make excuses for her staff. Demetria also called and stated, ” I don’t think Michaela told you that without her first calling the doctor”. More excuses!!! I did have a wonder nurse to come into my mom’s life and mine within the last week of her life. Her name was Antoinette. She was wonderful. She would call and say, I’m on my way. She would never call and say, do you need a visit? She took care of everything at the end and we were grateful. We also had a wonderful pastor name Michael. He would come out to our home and pray with us and he was a true friend to our family. I ask him to do my mom’s eulogy and he accepted. You don’t find good people like Michael and Antoinette anymore. We cherish the positive interactions we had with these 2 wonderful people. Thank you Michael and Antionette!!! The on call nurses at Brighton Hospice are not trained to take emergency calls from family members. They need extensive training and management is refusing to knowledge that there is a real problem.
Sincerely,
Natasha Tucker-Grays

The care we received was fine. However, I asked several times to not be called by hospice after my fathers death and Brighton did not respect my wishes hence the 2 star review. (They stay connected with family for 1 year after the passing of a loved one). For some, I’m sure this service is helpful but for me it opened up old wounds and trauma and I desperately wish they would have respected my request.

I am deeply grateful for the support my family and I received from Transitions Care during this very difficult time. The care and compassion shown by Daria, the nurse, and Jo, the social worker, made an immeasurable difference.
Daria’s professionalism, warmth, and steady presence brought comfort and reassurance when we needed it most. She not only provided excellent clinical care but also treated my mother and our family with dignity, patience, and kindness. Jo’s guidance and compassion helped us navigate the emotional challenges, offering strength and understanding at every step.
Without their dedication and support, I cannot imagine how we would have made it through this period. They were more than just members of a care team. They became a source of strength for our family.

Jo, LSW and Daria, RN are two of the kindest and most involved providers I have had the privilege experiencing. They go above and beyond for not just their patients, but the families as well. Due to their involvement Transitions hospice is a referral I would give to anyone who has a loved one on hospice. These two truly encompass compassion, collaboration, and respect in all they do!
Thank you Transitions for changing the narrative around hospice care!

When it was time for our Mother to enter Hospice, the Doctor’s and other caregivers gave us names of companies that manage the process for patients like my mother. We interviewed 3, and Mom selected Transitions because their people were attentive to her wants and needs, and we had excellent references from friends that had used the company for their loved ones.
Our Mom had great physical and emotional care from the moment she said yes until she passed away weeks later.
The communication with our family was terrific, and has continued even after Mom’s passing.
I highly recommend Transitions.

Transitions was FANTASTIC! Mom had cancer and was doing well until she had an unanticipated stroke. She survived 12 days post-stroke. We called hospice about 5 days in, so we only used Transitions for a week. They took such better care of her than the nursing home on their own ever did and best of all they spent the last 4 nights of her life with her….they were there with her ALL NIGHT, so we didn’t feel like we had to be there every minute with her. It was such a blessing! They communicated w/us along the way, even with the bad news. I received a call about 2:15am and our hospice nurse said it might be a good idea to head to the nursing home, as signs were pointing to her passing being very close. As I was leaving the house, I received another call that I was too late. We still went to give mom hugs and Shelter (our hospice RN through most of this) was there ready to give my sister and I hugs, as well. We finalized the pre-planned details (take the outfit with her, etc) for the funeral home to pick mom up with Shelter and she continued to wait with mom until they did so. I was grateful that mom wasn’t alone when she passed, so thanks to Transitions for that. I would highly recommend their care!

I was thankful for the love and support that the Transition team showed my family. It is never easy to move a loved one to a care and facility of this type. They showed us genuine care, from the initial visit from Sue at the hospital to my mother and her supporting family. Over the next few weeks, her support staff visited us to provide care and support, which eased her pain and took care of all her needs. When mom eventually passed away, Transition support for our family did not end. They continued to support our family by checking in on us and adding much-needed support and guidance to help us move forward. No one ever wants to recommend hospice care to a friend or loved one, but if I had to, I would pull Sue’s business card and the information for this wonderful service.

Compassionate, caring, dedicated, professional, amazing. These are just some of the words I would use to describe the care givers at Transitions Care. The nurses, Chris, Danny, Clint, Kristy, Bridget, and others, all went above and beyond to be kind, patient, and answered all concerns with professionalism and skill. The office staff, Susie, Tammy and others were always able to help problem solve and find resources that we needed. I am sure I will never spell her name correctly, but the nursing assistant who helped care for mom, was incredible. She was kind and gentle and patient with all of us.
If we could give 10 stars, we would. Transitions Care is the best.

Our family was fortunate to have been introduced to Transitions Hospice Care through our family friend, Susan King. My father was seriously ill and we needed help. As soon as Transitions became involved, my father received immediate professional medical care. As a nurse, I was impressed with the quality and through attention that was given to all areas of his needs. This was done with compassion and complete respect for his dignity. Our family was involved with the plan of care at all times. We were able at any time to call with any questions or concerns. As we needed a hospital bed, Wheelchair, Oxygen, and basic supplies Transitions had them delivered the same day and when my father passed away Transitions removed the equipment within hours which was an emotional benefit to our family. The two nurses Mary and Jenelle from the Fort Wayne office not only gave excellent medical care but were emotional support to us all. Please get in touch with me for you and your loved one.

This team is amazing. Every single person was caring, friendly, helpful, compassionate, efficient, and the absolute best advocates for my dad. They made the process seamless, stress free, with the utmost respect for me, my family, my dad, and everyone we worked with from the hospital, transportation, to the facility he was taken to. They were very responsive. We never felt alone. They educated us on what to expect along the way so there weren’t any surprises. My father passed extremely peacefully with the best care team I could ever dream of having for him. They even helped me navigate the funeral home questions and have continued to check in on us even after he passed. I cannot say enough good things about this team. They made one of the hardest times of our lives so much easier.

Paradigm Health was truly a blessing during the final weeks of my mom’s life. I didn’t initially think she would qualify for hospice care, but to my surprise, she did — and the support began immediately. That very same day, Paradigm arranged for a hospital bed to be delivered, which made an incredible difference in keeping her comfortable.
They provided a generous supply of incontinence products and essential comfort and pain management medications — all at no cost. These resources were not just helpful; they were vital in ensuring my mom’s last days were as peaceful and pain-free as possible.
Every single person we interacted with from Paradigm felt like they were made to work in hospice care. Their compassion, empathy, and calm presence made a profound impact — not just on my mom, but on our entire family. They handled everything with such grace and dignity, and they knew exactly how to support loved ones during such a difficult time.
One of the most touching moments was when they arranged for a singer and guitar player to quietly perform during my mom’s final days. It was a beautiful and peaceful way for our family to say goodbye, and it’s something I will never forget.
I can’t say enough good things about Paradigm Health. Thank you for being there for my mom — and for me. Your care will always be remembered.

I would give this company 0 stars if I could. My friend signed up with them when she found out her condition was terminal. They provided some of the equipment but in two weeks they only came out two times. There were no gloves provided and no real help. There was no real support to help her caretakers (we are just lay people-not nurses)make her more comfortable. Also the meds she was supposed to get didn’t come for days. When the end was near they wouldn’t make the arrangements to move her to a nursing facility (she had signed up for that) because her sister in law was calling and not her (at that point my friend was unable to call). The sister in law finally had to contact a friend who was a nurse to help. It was a complete nightmare and I wouldn’t use this company if it was the only company in Indiana providing this service. We got through this situation but with no help from the company that was supposed to assist us.

My mother in-law had this company for hospice and they were a nightmare. We changed companies because they did not ever provide enough supplies for her care. The family continually had to call the company for diapers, wipes, chux, and gloves. They also stated that an aide was going to come 2 times per week which did not happen. We are in the process of reporting this agency to CMS for not providing what is expected for end of life care. Terribly sad.

Life’s Journey Hospice provided compassion, clear communication, and steady support from the first call. The team listened to our needs, explained options with patience, and collaborated respectfully with our family. Nurses and aides offered gentle care and attentive comfort, while the physician ensured pain and symptoms were managed with dignity. Their presence created a calm, dignified space during a difficult time, and they respected our loved one’s preferences, routines, and values at every step.
In addition, Life’s Journey Hospice offered practical help and emotional support for our family as the illness progressed and after passing. The team checked in regularly, helped with arrangements, and provided resources for grief, making a stressful time more manageable. What stood out was their respectful pace, honest conversations, and dedication to patient comfort and family guidance without judgment. My family recently used Life’s Journey for hospice respite entrusting my wife with severe dementia to their care. Her experience was disastrous. There was no air conditioning throughout her visit. The humidity in the room was so high, that the electronics including her bed quit working and she had to be moved to another room her last night. Her medications were not administered properly resulting in her experiencing a catatonic state of consciousness during most of her stay. Family members and visiting friends were appalled at her state of being. She was only gotten up out of bed and dressed on the first full day. My daughters who visited her daily found her unresponsive and on one day having been left and the door closed as she lay in her own excrement. When I arrived to bring her home, she was in a catatonic sleep at 11 am and didn’t begin to recover for 36 hours. I would not recommend this facility to any family needing respite care for their loved one with advanced dementia.

Update:
Okay after a tough start we were all on the same page eventually. I’m happy She was able to pass in peace without pain! I realize we all have room for improvement so I’ve revised my report. Thanks to all who helped us through this!
No Stars for Life’s Journey! Came recommended by the hospital staff but I would NOT recommend. Patient is EOL (End of Life) they shouldn’t be suffering in pain waiting for Life’s Journey’s left hand to communicate with the right hand. Prayers out to everyone else that’s experiencing the same.

I had a terrible experience here with my father and wish I wouldn’t have brought him to this place. Unfortunately I lost my dad. The nurse Mark failed to give my dad his inhaler and argued with him about having it when it was in the medicine cabinet the entire time. Then my dad had to call me because Mark wouldn’t give him his medicine to help with his breathing/anxiety and stated that my dad had to ask for it which was completely not true. It was an “as needed medicine” and How does a sick patient know to ask for a new medication when they take a fist full daily and depend on the facility to help them. I got there and the nurse Mark argued with me about it as well. Totally unprofessional. Well after this incident within 12 hours my dad went unresponsive and never woke up. Shame on you!!! Do not take your loved one here unless you plan on not taking them home. Don’t get me wrong there are other good nurses there but the bad outweigh the good since my Dad is no longer with me.

I can’t say enough great things about Life’s Journey Hospice, the people who work there and the owners!
We started with Palliative care for out dad, he just wasn’t done just yet with his chemo. They were amazing! Our Palliative nurse practitioner took the time to understand dad’s needs and made him feel comfortable.
When it was time for dad to move into hospice, he was ready and understood that he wasn’t dying right now, that he just needed more care and support and so did we. It was amazing to hear him say these words himself. I give all that credit to Life’s Journey and their team who helped all of us understand what we didn’t know before.
The Hospice team was amazing! And he even got to go to their hospice house for a respite stay! And that was included in the hospice benefit! WHO KNEW!?!?!?! Well thanks to Life’s Journey, we now knew. We are so grateful to them for their compassion and helping us understand all the benefits of hospice. Our dads quality of life is better than we could have ever imagined!
We wanted to give them this public shout out because we almost didn’t know anything about them and we didn’t want another family to potentially miss out. We were given “choice” at the hospital, but choice was 4 pages long and we had no idea where to even begin. “Someone” wink wink, pointed us in their direction and we “interviewed ” a couple others……well we had pushy, we had the big company and then we had Life’s Journey. Our Life’s Journey navigator was genuine, took her time to ask us what we needed and then compassionately spent almost 2 hours with us. You could just feel the empathy and genuine desire to help us in everything she said and did. That sealed the deal for us!!! And she and the Life’s Journey team has lived up to every word!
Our mom is not in great health and our Navigator at Life’s Journey heard how we were struggling there too and helped get us an amazing home care company, a service where the physician care comes to us!!! Yes, comes to us!!!! Mom gets Palliative care now too with Life’s Journey and we can’t sing their praises loud enough!!!!!

I found Life’s Journey by accident one day when my 88yr old father got lost and ended up at their doorstep, confused without his cell phone. They took him in, made him comfortable and performed detective work to find me so I could come get him. Three weeks later he suffered two strokes and I knew Life’s Journey was the hospice I wanted him at. They treated him with such kindness and respect that I knew it was the right place for him to finish his life’s journey. I can’t thank them enough for the wonderful compassionate care they gave him and my family. Thank you to Mindy, Veronica, Rick and the others whose names I can not remember. You truly were a gift from God.

The team and staff at Franciscan Hospice House took extraordinary care of my sister in her final months. They were kind, compassionate, and professional every day and always made my sister and her family comfortable and welcomed. I cannot express in words my thanks to them adequately. I will always feel a debt to them and will recommend the location to anyone else caught in a difficult end of life situation.

The facility is comfortable and lovely. But the kindness and care of the staff is what makes this hospice home a blessing. I do not have adequate words to express what their kindness meant to my husband and me. I think I needed the oasis provided almost as much as my husband did.

My Mother went here and the care she received was the absolute best. I stayed with her every night and she thought we were on vacation together!
Every person that helped her was the most caring, helpful and sweetest to both my Mom and me. She passed away Feb.10. We could not have had a better or more peaceful experience. I will remember the love and peace we shared, not the sickness.

What can I say? My father went in hospital December 18, 2020. We found out he had cancer a week before. He had a stroke 5 days while he was in hospital. Transferred to rehab after that then to Franciscan hospice house. I was heartbroken but my dad was well taken care of. Everyone was so nice they always answered my questions. I could call anytime day or night to check on him. During this time with Covid I felt they were fair in their visiting hours. It’s a rough time for everyone. They kept my dad comfortable and that’s all that mattered. Our nurses were wonderful! I don’t know how they do it! If you are looking for a place for your loved one this is the place.

The care and compassion provided to my mother was exceptional. The staff- angels on earth. Each member of my mom’s care team treated her with gentle care, respect and dignity. Additionally, that same kindness was extended to our family. How fortunate we were to find this facility in our time of need.

Our family was very pleased with Heartland in Muncie. First, Tammy helped initiate the service and was so helpful explaining everything, Sara and Emily were our wonderful RNs and Amber was a very compassionate NA. Tammy the social worker was so knowledgeable and sweet and the Chaplain Andrew was always available if we needed him. They all were excellent in this extremely sad time for us.

They messed up from the get go. They failed to prescribe necessary medications, which kept my mom very uncomfortable. They also failed to get home health equipment delivered on time and failed to coordinate my mom’s discharge from the hospital in timely manner. They didn’t bring any supplies that the nurse told me we needed thus making my mom’s last days more uncomfortable and leaving her to suffer more than necessary. Would NOT recommend!

Do not use these people they aren’t there to “make them comfortable” they will just hasten your loved one to the grave. Started calling pharmacies before they even went into the room to look at my grandpa then prescribed way too much morphine and got upset when we called for help, showed up up to hours late to help with an attitude in a time of distress. My grandpa was completely coherent and aware one night before they upped his dose then he was in a comatose state they said this is what we want and he died the next day. That was a Sunday the following day on Monday a box truck knocked on the door at 10:30 am to pick up all the medical equipment that showed up too late anyway. These people are sick and do not care for the people they claim they do their goal is to get them in and out as fast as possible so they can make more profit and it’s sick you won’t ever take credit for it and I couldn’t get lawyers good enough to prove it but I know and you know you guys killed my grandpa he had cancer but he died from your dosage

My mom passed away on 10/21…. I am happy to write this review and tell others what a positive experience we had with Heartland. When our referral was made to Heartland, we had an Intake within a day, nurses out the following day and the intake was complete. We met with several wonderful and caring individuals, each providing a different level of support…chaplain, social worker, aide, nurse etc. When they said they would do something, they did it. When my mom needed meds, she got it. They offered their services 24 hours a day, and they meant it. They were loving and kind to my mom and to me. Nurses were very knowledgeable and always willing to answer our questions and honesty and respect. Nurses were always willing to sit and talk if I needed it or just hold my moms hand towards the end. Heartland was a great support to my mom and our family and I would highly recommend them to anyone!!

My mom passed away on November 2 of this year and I am so grateful for this company. I switched to Heartland because of a bad experience with another company. I called Heartland in the morning , and by afternoon they had everything set up for my mom….all supplies, including changing her bed to something more comfortable. All completed in a day. Gloria, the nurse, was absolutely wonderful; the responses from the company were almost immediate….including a call to them at 2:00 am when I needed some help with a nurse in the facility where my mom was who was not medicating her correctly. The last days of my mom’s life were made so much better by this kind and caring group of people. It was truly a perfect experience in a trying time. Many thanks to them.

Words alone can’t express how amazing this organization is. My mother was on their service for almost a year.
They provided my mother and my family with everything we needed and more to care for my mother in her final year.
My mom started out in a nursing home and they assisted us in transitioning her to home so we could keep her close.
When home care was no longer possible they assisted with getting her into a proper facility.
While in the nursing home it was such a relief knowing there was a set of independent eyes looking over her and advocating for her needs when we couldn’t be there.
Her Nurse Rachael was just incredible she made herself available 24/7/365; even when she was on vacations she coordinated my mother’s care.
Yesterday, was my mother’s final day on earth and Rachael stayed by our sides the entire day to make sure she was comfortable and at peace.
I can’t express my gratitude enough for the love, compassion, professionalism her entire care team provided.
Especially Rachael you will forever be in our hearts.

I would recommend NOT using this Company when looking for end of life care for your loved ones. They did not supply everything for my Mom’s care and even after being told my Mom would not last the weekend they still didn’t bring the pain meds to keep her comfortable, but instead a friend of our family had to help us find some pain meds. Sad since this is what they claim to help your loved ones during this time!!!

I recently had to commit my Mom to hospice. I’m thankful that I chose compasses. They were so kind and took good care of my Mom until she passed 06/21. I feel like God has this really big garden with fluffy, colorful flowers. And when there is a need God picks the flowers that are the beautiful souls that is the staff at compasses. I didn’t meet not one person who wasn’t worthy of this review. The grounds are beautiful with in and out access to the patients rooms with a beautiful garden view. It was quiet and peaceful. Thank you Compasses staff for making my Moms transition comfortable and ok for me and my family. RIP Kathryn Eiland-Guthrie.

I wanted to express my personal gratitude for the effort and support that Compassus contributed during the recent stay of Bob.
The facilities were lovely and provided a comforting and comfortable place for us as his family and friends. The facility was quiet ( no hospital vibe) and warm with many options provided by staff including the families choice of music, snacks, etc. The views out the french doors, the birds and cozy fireplace setting surrounded us with a true feeling of comfort and support. We were there during a sunny time, which Bob was able to enjoy since there was an entire wall of windows/doors. The staff said the beds can be wheeled out the doors to allow all to enjoy the sunshine. Practical considerations like the a full kitchen and small fridge in the rooms, were thoughtful and helped smooth out our stay.
The staff were all so professional and supportive….. some almost felt like extended members of our family. They were always there to look after Bob and also us, with kind words and helpful suggestions. The invaluable support of the volunteers, kept the bird feeders full even during brutally cold weather and
those sewing unique bed gowns offered a daily opportunity for a smile.
With sincere appreciation…..

I cannot say enough about the care my son Jeff received at Compassus. I came to hospice looking for relief from months of trying to find solutions to his health issues. When it became clear that solutions were not going to be found we came to Compassus. In Comassus we found people who helped us to understand and accept what was happening to Jeff; helping us to find closure. Jeff was comfortable, cared for, and in the end safe.
Julie, his nurse, clearly explained what was going on with Jeff, so we could understand what Jeff was experiencing. All of the staff who gave him care talked about what they were doing and why they were doing it. We felt included.
The physical setting of the facility was beautiful. His room was calm and quiet. The view from the French doors was astounding, even on a gloomy January day. There were squirrels running up and down the partitions on the patio, garden plots, and winding paths. It was very comforting to take a break and walk around.
Christopher the Chaplin was special to us. Knowing he visited with Jeff was so important to us. The competent staff and calming atmosphere gave us the peace we needed during a difficult time.

There is no way my sister and I would have endured the process of my mother’s passing without her end of life caregiver, RN Mark. The care at Compassus was wonderful. The caseworkers and the staff went out of their way to ensure my mother, as well as my sister and I, were as comfortable as possible. The grounds and gardens are beautiful and peaceful. I was so grateful to Mark for providing me with Barbara Karnes booklet “Gone From My Sight”, which I still go back to over and over, even a month after her passing. This booklet is SO important if you have a sacred person nearing the end of life. I cannot recommend it enough (and her Instagram page “barbarakarnesrn”) and I only wish I’d had it in the months prior to my mothers passing. All the signs were there, but I didn’t recognize them. I learned there is a roadmap of sorts, an unfolding, to the process of dying. We were so emotional, raw and often terrified during my mothers end of life journey. I’m so grateful to God that although deeply painful for us, my mother’s passing was peaceful for the most part, and I credit RN Mark for ensuring hers and our comfort during that most difficult time.
Mark, thank you for being such a kind and compassionate human being. Thank you for taking care of my mother Judy, and for taking care of us. We learned so much from you and we will never forget you. Never.

I have had a family member in this facility before when it was St. Vincent’s. I am not sure about the name change but the facility itself was gross in comparison to 15 years ago. Apparently maintenance and cleaning are not in the budget, which is sad given the cost. I cleaned the dirty dishes in the kitchen but not the filthy floor. The ceiling tiles by the ceiling vents were gross and there were cobwebs. There was a recliner from the turn of the century with a shredded foot stool. The RN was kind and caring.

There really are no words of the overflowing care and compassion that is given by every single one of the employees from heart to heart . My mom has the best care hands down . Not only do they care for mom but also they care for me and making sure I get time for myself. My brother passed away and they came to funeral home and made sure my mom was ok and they sat with her the entire evening .The CNA’S that come and do her bathing and make sure all her needs are met are outstanding and very compassionate in making my mom comfortable and very clean . My mom loves them . Just to sum it up big hearts ❤️ to all of them from all of us to them

I have been taking care of my father for 5 years and we used a hospice facility that is affiliated with a hospital in Indianapolis.
We switched the Heart to Heart facility and the level of care, compassion and knowledge that this team has is remarkable and they go above and beyond their duties to keep your loved one comfortable during your time of need.
Not only were they concerned about my father’s comfort but they were also concerned about mine as well.
As far as I am concerned these nurses are angels suffer from heaven.
The case managers and nurses took time to explain every step of the process and that is what made this so much easier for me.
With deep gratitude and appreciation I thank you Heart to Heart for the love and compassion that you had for my father to the end of his life. 8/15/24

I have no complaints about the nursing staff all were wonderful. My complaint is with the doctor. On one occasion the doctor didn’t sign a script for my father for medication and I had to call the next day to find out why the pharmacy had not received the script yet. That was minor compared to what happened after my father passed. My father passed on Feb 2, 2024 and on Monday Feb 12 the funeral home called me and notified me that the doctor still had not signed the death certificate (10 days later) . The funeral home had reached out to them twice. Finally that day after I had called irate it got signed. It is a shame my father had to lay there for 10 days before he could be cremated.

Given the wonderful care this company gave my grandmother 5 years ago, I chose to use them again for my great aunt. She has been in hospice a few times with them but this time around I was highly disappointed. The day before she passed she was in distress and needed orders from Heart to Heart for comfort medication. I was told by the nursing facility someone came the next day to write the orders.
At the time of this post, it’s been 9 days since my great aunt passed away. No calls, no nothing. When my grandmother passed, someone came to the facility to check on my mother and I. This time, nothing. Only the chaplain, which was very kind of him.
I will not use this company again. Really disappointed. I will always give the utmost respect and praise to those who deserve it. Unfortunately this time I can’t.

When my mother needed hospice we were told that Heart to Heart was one of the best by the facility she was in. If they are one of the best I would hate to see the worst. Every thing we were told they would do they did not do. We were never notified of her condition after a visit which was a huge thing to our family. The day she died I had to call to get a nurse to check her. We were never told she was actively dying. We would have done so many things differently had we known. The whole family was not able to be with her due to their neglecting to tell us. There are too many more things to list. Please do not use this company for your dear family member.

Frequently asked questions about hospices in Indianapolis
What is hospice care, and how does it work in Indianapolis?
Hospice care focuses on comfort, quality of life, and support for patients with a terminal illness. In Indianapolis, hospice programs operate under state and federal guidelines and are often Medicare-certified providers. A typical plan begins with a referral and an assessment by an interdisciplinary team that includes a physician, nurses, social workers, chaplains, and trained volunteers. The goal is to manage pain and symptoms, support families, and respect patient preferences. Care can be delivered in the patient’s home, an inpatient hospice unit, or a compatible facility, depending on need. Regular care planning meetings help tailor the plan to changing conditions. If life expectancy estimates change or symptoms intensify, the care setting and services can be adjusted accordingly.
Who is eligible for hospice care in Indiana?
Eligibility for hospice generally requires a terminal diagnosis with a prognosis of six months or less if the illness runs its typical course, as certified by a physician. In Indiana, a patient must elect hospice services and agree to focus on comfort care rather than curative treatment. Eligibility is assessed by the hospice program and is often supported by documentation from the patient’s primary clinician. Medicare, Medicaid, most private insurances, and VA benefits commonly cover hospice, though coverage specifics can vary by plan.
Where can hospice care be provided in Indianapolis?
Hospice care is often delivered in the home, which enables patients to stay in a familiar setting with family members and pets nearby. When home is not feasible, Indianapolis hospices may offer an inpatient hospice unit or care in a partner facility or hospital. The care setting is chosen based on symptom burden, safety, and caregiver capacity, with the 24/7 on-call team ready to respond to changes in condition.
How is hospice care paid for in Indianapolis?
Most hospice services in Indianapolis are covered by Medicare Part A, Medicaid, or private insurance, often with little or no out-of-pocket costs for routine care. Families may also have options for private pay or long-term care benefits. Coverage typically includes medical supervision, medications related to end-of-life care, nursing visits, social work, chaplain services, bereavement support for families, and equipment such as beds or ramps. It’s important to review the specific benefits with the hospice program and the insurer.
What services does hospice include?
Hospice care is delivered by a multidisciplinary team that provides pain and symptom management, nursing care, emotional and spiritual support, social work assistance, bereavement services for families, and volunteer support. The plan also covers care coordination, caregiver education and respite care, and access to equipment to maintain comfort and safety. Regular reassessment ensures the plan stays aligned with patient goals.
What is the difference between hospice and palliative care?
Palliative care focuses on relieving symptoms and improving quality of life at any stage of a serious illness, and it can be provided alongside curative treatment. Hospice care is a form of palliative care reserved for those with a terminal prognosis and a plan to discontinue curative efforts. The two share the goal of comfort, but hospice typically includes a formal end-of-life care plan and is often covered by Medicare after meeting eligibility criteria.
How do I choose a hospice provider in Indianapolis?
When selecting a provider, consider licensure, accreditation, and the provider’s experience with the patient’s specific condition. Look for 24/7 on-call availability, a transparent care plan, clear communication, and strong support for families. Reading reviews, requesting references, and visiting a potential facility can help determine if the provider’s values and approach align with the patient’s preferences.
What should families expect during hospice care?
Families can expect regular visits from the interdisciplinary team, ongoing care coordination, and support to manage symptoms at home or in a facility. The care plan will include medications for comfort, instructions for caregivers, and access to bereavement support for loved ones. Education about what to expect as illness progresses and guidance on making advance directives can help reduce uncertainty during stressful times.
What happens after hospice care ends?
After the patient passes, many hospices provide bereavement support for a designated period, often including counseling, group support, and resources for families coping with loss. Some programs offer ongoing grief counseling and memory resources for a set time, while others tailor support to family needs. The hospice team can also assist with practical steps after death and coordinate with funeral providers as requested by the family.
How can I start the conversation about hospice with a loved one in Indianapolis?
Starting the conversation about hospice care should be compassionate and patient-focused. Involve the patient’s physician or trusted clinician, discuss goals of care, and address fears about loss of independence. Provide clear information about what to expect, emphasize comfort and quality of life, and allow time for questions. Family members can share the plan gradually, and you can revisit the topic as the illness evolves. Early conversations can help ensure decisions reflect the patient’s values and wishes.
